If you are unsure of how to complete any of the information requested, our Membership personnel would be happy to help you in person. Note: The form (and requested changes to your Membership status) is not considered complete until signed by SVAC personnel. You may fax the completed form to 460 9900 and we will call you to confirm that the transaction has been completed.
